Some collaborations are born overnight.  Others are forged over years of vision, persistence, and a shared obsession with excellence.  The partnership between L’Atelier Forte and Dr. Apa falls firmly in the latter category.  What began as a cold call in 2021 has evolved into a multi-year creative journey, uniting fashion and aesthetics at the highest level to change the way the world sees doctors.  At the core of this collaboration was The Apa Bomber, but L’Atelier Forte x Dr. Apa is not just the story of a jacket.  It’s the story of a relentless pursuit to bring purpose, precision, and style to the office.  Now, four years later, the next chapter begins with the launch of The Apa Bomber, V2.

May 2021: The call.
L’Atelier Forte cofounder Joe Vasold made a cold-call to Dr. Apa to request a meeting.

June 2021: The meeting.
Dr. Apa reviewed Forte’s product line. The fabrics were special but the styles didn’t fit the Apa aesthetic.

October 2021: An email from Joe to Dr. Apa.
New designs were in development.

November 2022: A prototype.
Joe returned to Apa Aesthetic New York, with a prototype of the Apa Bomber. It took 3 months, multiple meetings and many more prototypes to perfect.

Do You Really Need to Shave Down Your Teeth for Veneers?

In the age of social media, it’s easy for misinformation to spread, especially when it comes to medical or dental procedures. One common misconception that’s been gaining traction lately is whether you need to have your teeth drastically shaved down to get veneers. Videos of patients with severely prepped teeth are going viral, sparking a wave of confusion and concern. Let’s set the record straight.

First and foremost: those viral images of dramatically reduced teeth are not preparations for veneers—they’re preparations for crowns. There’s a big difference between the two and understanding it can save you from unnecessary stress and potential harm.

Veneers vs. Crowns: What’s the Difference?

A veneer is a thin shell of porcelain or composite material bonded to the front surface of a tooth. Veneers typically require minimal preparation, often involving the removal of just a small amount of enamel to ensure a proper fit. On the other hand, crowns cover the entire tooth and require significantly more preparation, often involving the removal of a substantial portion of the tooth structure.

Why might someone need crowns instead of veneers? Crowns are typically used when a tooth has extensive damage, such as severe decay or structural compromise. In contrast, veneers are primarily cosmetic and ideal for enhancing the appearance of healthy teeth.

The Importance of Preserving Enamel

One of the biggest concerns with over-prepping teeth is the unnecessary loss of enamel. Your teeth consist of three main layers:

  1. Enamel: The hard, outer shell that protects the tooth.
  2. Dentin: The softer, more porous layer beneath the enamel.
  3. Pulp: The innermost part of the tooth that houses the nerve and blood vessels.

Enamel is the strongest, most durable part of the tooth, and preserving it is crucial for long-term oral health. Why? Because the bond between a veneer and enamel is much stronger than the bond between a veneer and dentin.

When a tooth is over-prepped and too much enamel is removed, the dentist is forced to bond the veneer to dentin instead. Dentin is porous and less dense, which can compromise the strength and longevity of the bond. Over time, this can lead to issues like leakage, decay, and even the need for root canals.

Why Over-Prepping Is Risky

Once enamel is removed, it can’t be replaced. This is why over-prepping is a serious concern, especially for young patients. Consider this scenario: A patient in her twenties opts for veneers and undergoes aggressive tooth preparation. Veneers typically need to be replaced every 15 years or so. This means that over her lifetime, she may need to replace her veneers four or five times.

Each replacement involves additional preparation, which gradually brings the dentist closer to the nerve. If the nerve is exposed, a root canal becomes necessary. Furthermore, if the original preparation wasn’t done meticulously—leaving gaps or “open margins” where the veneer doesn’t fit snugly—decay can develop underneath.

The Long-Term Implications of Poor Preparation

If a veneer or crown isn’t properly fitted, it can start to leak. Leakage occurs when bacteria and saliva seep into the tiny gaps between the restoration and the tooth. In dentin, which is more porous than enamel, this process happens quickly. If left unchecked, the underlying tooth can become decayed or infected, potentially requiring extensive (and expensive) repairs.

That’s why precision is so important. A well-fitted restoration should be sealed perfectly, with no gaps or rough edges. Regular dental checkups are also critical to catching any issues early.

When Is Aggressive Preparation Necessary?

It’s worth noting that there are legitimate situations where more aggressive preparation is unavoidable. For example:

  • Severe decay: If a tooth has significant decay, a crown may be necessary to restore its structure and function.
  • Old restorations: Teeth with old crowns or large fillings may require additional preparation to accommodate a new restoration.
  • Structural damage: In cases of cracks, fractures, or extensive wear, a crown might be the best option.

However, these cases are the exception, not the rule. For healthy teeth, minimal-prep veneers are almost always a better choice.

When a patient walks into the dental clinic seeking a better smile, the process of transforming their vision into reality involves a delicate balance of creativity, technical skill, and personalized care. Each smile is unique, and achieving the perfect result requires an understanding of both the patient’s expectations and the artistic elements that define their ideal smile.

Understanding the Challenge

When a patient requests a better smile, the initial step is to assess their current dental aesthetics and identify areas for improvement. In some cases, the patient may not provide specific directions or expectations, which can be both an advantage and a challenge. Without explicit preferences, we have the creative freedom to design a smile that enhances their natural features, but it also means we must carefully interpret the visual and functional elements to meet their implicit desires.

In the case at hand, the patient’s primary request was for an improved smile, but no specific guidelines were given. Our initial observation revealed several issues:

  1. Tissue Heights: There was a noticeable difference in tissue heights between the two central incisors (teeth #8 and #9). To align the central incisors correctly, hard tissue crown lengthening would typically be required. However, given the patient’s busy schedule and the complexity of this procedure, we decided to explore alternative methods.
  2. Incisal Edge Position: The incisal edges (the biting edges of the teeth) were not aligned, with visible wear indicating teeth grinding. The patient also had a missing canine and large fillings in some of the lateral teeth, which contributed to the irregularities in the smile.
  3. Negative Space: The right side of the patient’s arch showed more negative space compared to the left. This discrepancy was due to the jaw position relative to the lip, affecting the overall smile appearance.

Crafting the Design

With these observations in mind, we devised a comprehensive plan to enhance the patient’s smile while addressing the functional aspects. Here’s how we approached the design:

  1. Laser Tissue Removal: Instead of hard tissue crown lengthening, we opted for laser treatment to remove excess tissue and reshape the gum line. This method allowed us to adjust the tissue heights without the need for invasive procedures. By carefully removing tissue and examining the bone heights, we aimed to create a more balanced and natural appearance.
  2. Incisal Edge Adjustment: To correct the incisal edge discrepancies, we planned to adjust the edges to create a harmonious balance. We decided to place the incisal edges between the two extremes observed—shortening the edge on the left and extending it on the right. This adjustment would help in achieving a more even and aesthetically pleasing look.
  3. Building Out the Smile: We needed to build out the smile on the right side to fill the negative space and create a fuller appearance. This involved adding material to the existing teeth to ensure that the smile appeared complete and well-proportioned.
  4. Addressing Lower Teeth: Given the impact of the lower teeth on the overall occlusion, we planned to make adjustments to the lower arch as well. This included repositioning the canines and ensuring proper alignment to avoid any interference with the upper teeth during movement.

The Transition to Permanent Restorations

During the temporary phase, we made adjustments based on the patient’s feedback and our observations. The temporaries helped in refining the three-dimensional position, volume, and shape of the smile. This phase is essential for identifying any issues and ensuring that the final restorations align with the patient’s expectations. 

Once the temporaries were finalized and approved, we proceeded with the permanent restorations. While temporaries provide a valuable guide, the final restorations are crafted with greater precision and attention to detail. The goal is to achieve a smile that enhances the patient’s natural features without deviating from their authentic appearance.

The final restorations were designed to closely match the patient’s original teeth but with improved aesthetics. This approach ensured that the smile looked natural and complemented the patient’s facial features. Achieving this level of refinement requires a deep understanding of dental artistry and a commitment to delivering results that are both functional and beautiful.

Our patient, a finance professional from Switzerland, faced a unique dental challenge. He was congenitally missing his lateral incisors, a condition that significantly impacted his smile and overall dental aesthetics. As someone actively involved in starting a fund and raising money, he desired a solution that would not involve the lengthy and often uncomfortable process of wearing braces. After consulting with multiple doctors and orthodontists, he reached out to our clinic, seeking a creative solution to his problem.

Understanding the Case

When the patient presented himself at our clinic, it was evident that the absence of his lateral incisors had altered his smile’s appearance. The primary goal was to create a natural-looking smile that compensated for the missing teeth. This required careful planning and consideration of several critical factors, including space management and the alignment of his dental and facial midlines.

Space Management

One of the first steps was to evaluate the space where the lateral incisors should have been. Proper space creation is essential to ensure that the final restorations look natural and function correctly. Our approach involved using Emax bridges with pink ceramic to create the necessary tissue contour, mimicking the appearance of natural teeth.

Evaluating and creating the necessary space involved several detailed steps. First, we had to determine the exact dimensions and positioning of the missing teeth. This required precise measurements and a thorough understanding of the patient’s existing dental structure. We used advanced imaging techniques to map out the area and plan the space creation meticulously.

Midline Alignment

Aligning the dental midline with the facial midline is crucial for aesthetic harmony. We measured the patient’s facial midline and planned to create a three-unit bridge that would include a lateral incisor and transform the adjacent canine into a proper tooth. Midline alignment is not just about aesthetics; it also plays a significant role in the patient’s overall dental health. An improperly aligned midline can lead to bite issues and other dental complications. This meticulous planning ensured that the final result would be visually appealing and functionally sound.

The Procedure

To address the patient’s dental needs comprehensively, we undertook the following steps:

  1. Creating the Bridge: We designed a three-unit bridge to replace the missing lateral incisor and convert the adjacent tooth into a canine. This bridge was crucial for creating the necessary space and alignment.
    Creating the bridge involved several stages. Initially, we crafted a detailed mold of the patient’s teeth to design the bridge. We then used Emax, a high-quality material known for its durability and aesthetic appeal, to construct the bridge. Emax’s strength and translucency made it an ideal choice for this procedure, ensuring that the final restoration would be both functional and visually appealing.
  2. Raising the Bite: The patient’s bite was collapsed, necessitating an adjustment. We decided to work on ten upper teeth, from the first molar to the first molar, and fourteen lower teeth. The lower teeth preparation was mostly prepless, except for softening the edges. We had to ensure that the new bite would be comfortable and functional for the patient. This involved adjusting the height and alignment of the teeth, creating a balanced and harmonious bite. The process was intricate, requiring precision and attention to detail.
  3. Using Emax Bridges: The bridges and restorations were created using Emax, a highly durable and aesthetically pleasing material. Emax bridges with pink ceramic helped us achieve the desired tissue contour and natural appearance.
    The use of Emax bridges was a critical aspect of this procedure. Emax’s properties allowed us to create restorations that closely mimicked natural teeth, both in appearance and function. The pink ceramic used in conjunction with Emax helped create a natural-looking gum line, further enhancing the overall aesthetic.

Final Results

Once the final restorations were ready, they were seated in the patient’s mouth, and the results were immediately noticeable. The pink ceramic and Emax bridges blended seamlessly with the natural teeth, creating a harmonious and aesthetically pleasing smile.

The process of seating the final restorations was delicate and required precision. We ensured that each restoration fit perfectly and aligned correctly with the adjacent teeth.
