  1. Tissue Heights: There was a noticeable difference in tissue heights between the two central incisors (teeth #8 and #9). To align the central incisors correctly, hard tissue crown lengthening would typically be required. However, given the patient’s busy schedule and the complexity of this procedure, we decided to explore alternative methods.
  2. Incisal Edge Position: The incisal edges (the biting edges of the teeth) were not aligned, with visible wear indicating teeth grinding. The patient also had a missing canine and large fillings in some of the lateral teeth, which contributed to the irregularities in the smile.
  3. Negative Space: The right side of the patient’s arch showed more negative space compared to the left. This discrepancy was due to the jaw position relative to the lip, affecting the overall smile appearance.

Crafting the Design

With these observations in mind, we devised a comprehensive plan to enhance the patient’s smile while addressing the functional aspects. Here’s how we approached the design:

  1. Laser Tissue Removal: Instead of hard tissue crown lengthening, we opted for laser treatment to remove excess tissue and reshape the gum line. This method allowed us to adjust the tissue heights without the need for invasive procedures. By carefully removing tissue and examining the bone heights, we aimed to create a more balanced and natural appearance.
  2. Incisal Edge Adjustment: To correct the incisal edge discrepancies, we planned to adjust the edges to create a harmonious balance. We decided to place the incisal edges between the two extremes observed—shortening the edge on the left and extending it on the right. This adjustment would help in achieving a more even and aesthetically pleasing look.
  3. Building Out the Smile: We needed to build out the smile on the right side to fill the negative space and create a fuller appearance. This involved adding material to the existing teeth to ensure that the smile appeared complete and well-proportioned.
  4. Addressing Lower Teeth: Given the impact of the lower teeth on the overall occlusion, we planned to make adjustments to the lower arch as well. This included repositioning the canines and ensuring proper alignment to avoid any interference with the upper teeth during movement.

The Transition to Permanent Restorations

During the temporary phase, we made adjustments based on the patient’s feedback and our observations. The temporaries helped in refining the three-dimensional position, volume, and shape of the smile.
